There are instances where the settings can go out after a while and or when one stops rapidly that the anti-reverse system does not kick in on engaging reverse.

As far as I know, the disengagement mechanism is not dependent on setting. I am not sure whether all brake systems are the same, but on my previous caravan, a 1984 Sprite, at least this was the case. I opened the hubs to see how it works. I don't doubt your assessment and Hennie's experiences, I am just surprised that this can happen.

When the person stops on a down hill it is natural that the caravan brakes would have started to work. If you reverse directly from that position the brakes will still be engaged.
You will have to pull the caravan on a level patch of road or even slightly uphill forward and then start to reverse. This way the system will allow you to reverse without the brakes engaging.
All this is possible only if the brake system is set up correctly.

You can also try to disengage the brake by pulling up the handbrake and then let the car go forward until the coupler is fully extended. Then fit a spacer to prevent the coupler pushing back once the handbrake is released. Best would be to fit the spacer before going down the hill slowly.
I have never seen these couplers for sale. The guys usually make their own with square tubing etc. Might be wrong but I think Lennie posted a photo of one.
I once used an old steering lock and it worked fine.
